For a minimum quantity of 500 sets, you may either submit your existing artwork or modifications of our and we will print your form on Carbonless Paper to the same specifications indicated above.

You may also combine two or more originals @ $75.00 per additional copy for a combined quantity-price provided that the number of parts is the same and that each combined quantity is at least 250.

Thus, if you submit artwork for 250 Kindergarten, 375 Grades 1-2 and 375 Grades 3-4 for a total of 1000 5-part sets, your price will be $476.00 for the base forms plus $150.00 for the two additional copy changes, totaling $626.00.
