Customize your own form to meet your needs!
Prices include printing your form to your specifications and as a result already include your school name and address if needed. We can even scan a drawing or your school logo to place on your forms to further customize them! There is never a charge for typesetting or artwork.
1. Decide what you want on your form. Create a rough draft of the content and either fax it to us (1-877-431-1201), email it to us (firstname.lastname@example.org), send it in with your mail order, or upload it with this online order (there are two upload fields above, one for your form, one for your logo). Even if you place your order online, you can still fax us the draft of your new form.
2. Choose the number of sheets you want per pad. This size form can be padded in stacks of 50 or 100.
3. Choose your paper color (this does not affect the cost of the form).
4. The minimum quantity is 4 sets for this pad size. It is always much more cost effective to order 100+.
5. Please include your contact information (fax or email) with your order so that we can send you a proof approval. New forms with generally require this step unless the changes are very minor. Once your receive your proof, you can make additional changes to your form. You will need to approve the final changes before your form is printed.
6. Please call for RUSH orders! Our normal turn-around is 2-3 weeks on custom orders but we can work with tighter deadlines if necessary at no extra charge.
